About ConnectedHealth Pharmacy
At ConnectedHealth, we’re re-inventing the pharmacy model. As a part of ConnectedHealth, you’ll work with a pharmacist who’ll consult with your doctor to design a program that is tailored for you, and connects your medication, nutrition, fitness and more. Your pharmacist will create a summary of all of your medications, their purposes, and how and when they should be taken.
Your pharmacist will also come up with options to help reduce side effects, trim your out-of-pocket costs or simply make it easier to stick with your medication. They will also work with your doctor to identify when you can stop taking medications as your health and wellness goals are met; and review your risk factors with your doctor so you can proactively manage your conditions.
